New Member Application

Applications for membership in TAAN are subject to review and recommendation by the Executive Committee and approval by a majority of the membership. All information submitted will be held in strictest confidence. (TAAN privacy policy)

To apply for Membership in TAAN:

1. Download and Open the TAAN New Member Application Word Doc.
2. Save the Word Doc file locally.
3. Complete the entire form. This document was designed so you could type your information directly into the form fields.
4. Attach the completed Application below, complete the form and send to the TAAN President for review. All fields are required.
